Key Responsibilities

• Provide direct patient care in a Long-Term Care / Skilled Nursing Facility setting
• Administer medications and treatments as directed by the care plan and supervising clinicians
• Monitor and document patient conditions, reporting changes to the appropriate clinical staff
• Collaborate with interdisciplinary care teams to support resident health and well-being
• Adhere to facility policies, procedures, and state nursing regulations

Required Qualifications

• Active, unrestricted Minnesota LPN or LVN license required — no temporary or pending licenses accepted
• Experience in Long-Term Care (LTC) or Skilled Nursing Facility (SNF) settings
• Ability to work rotating shifts as scheduled
• Must provide a minimum of 2 hours notice prior to shift start for any cancellation

Preferred Experience / Skills

• Prior PRN or per-diem nursing experience in a similar care environment
• Familiarity with LTC/SNF documentation and electronic health record systems
