Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based

Key Responsibilities

• Provide skilled home health nursing care to patients in the West Portland area and surrounding communities
• Conduct patient visits in the field, with up to 6 patients visited per day and a standard productivity expectation of 30 points per day
• Complete clinical documentation using EPIC Remote Client and Rover mobile application, including OASIS assessments
• Manage daily patient assignments with travel routes organized to minimize drive time; occasional total daily drive time may reach up to 60–90 minutes
• Deliver care in accordance with individualized patient care plans, maintaining a 1:1 care model per visit

Required Qualifications

• Active Oregon Registered Nurse (RN) license required
• Minimum of 1 year of skilled home health nursing experience
• Current Basic Cardiac Life Support (BCLS) certification
• OASIS documentation experience required
• Reliable personal transportation required
• COVID-19 primary vaccination series plus one booster required

Preferred Experience / Skills

• Familiarity with EPIC Remote Client for home health charting
• Experience meeting productivity point expectations in a home health environment
• Comfort navigating a broad geographic service area with varying patient locations
