Contract | Full-Time | Days (5x8 Hours, 8:00–16:30) | Onsite / Field-Based
Key Responsibilities
- Provide skilled home health social work services to patients in the East Portland area and surrounding communities
- Conduct home visits, seeing up to 6 patients per day, with scheduling designed to minimize drive and windshield time
- Complete OASIS assessments and documentation using EPIC Remote Client and the Rover mobile application
- Coordinate patient care plans and connect clients with appropriate community resources and support services
- Maintain accurate and timely clinical documentation in accordance with home health standards
Required Qualifications
- Active or pending Licensed Master Social Worker (LMSW) licensure
- Minimum of 1 year of skilled home health experience
- OASIS experience required
- Basic Cardiac Life Support (BCLS) certification required
- Must have reliable personal transportation; occasional daily travel radius of up to approximately 60 miles
- COVID-19 primary vaccination series plus one booster required
Preferred Experience / Skills
- Familiarity with EPIC Remote Client for home health charting
- Experience using the Rover mobile application for point-of-care documentation
- Comfort working independently in a field-based, community-focused care setting
