Contract | Days | Full-Time | 4x8-Hour Shifts (8:00–16:30) | Onsite / Field-Based

Key Responsibilities

• Provide skilled speech-language pathology services to patients in a home health setting across the west Portland area and surrounding communities
• Conduct patient evaluations and deliver individualized treatment plans in a 1:1 care model, visiting up to 6 patients per day
• Complete OASIS assessments accurately and in a timely manner as required by the home health setting
• Document patient care using EPIC Remote Client and the Rover mobile application
• Manage daily scheduling and travel efficiently within an assigned geographic service area, with occasional drive times of up to 60–90 minutes per day

Required Qualifications

• Active Oregon Speech Language Pathologist license
• Minimum of 1 year of skilled home health experience
• Demonstrated OASIS experience (must be disclosed if not applicable)
• Current Basic Cardiac Life Support (BCLS) certification
• COVID-19 primary vaccination series plus at least one booster required
• Reliable personal transportation required for daily patient visits

Preferred Experience / Skills

• Familiarity with EPIC Remote Client for home health charting and the Rover phone application
• Experience meeting productivity point expectations in a home health environment (standard expectation of 30 productivity points)
