Contract (13 Weeks) | Full-Time | Days | Monday–Friday 8:30AM–5:00PM with Alternating Weekends | On-Call Rotation Required

Key Responsibilities

• Provide skilled nursing care to home health patients including Start of Care (SOC), Resumption of Care (ROC), recertifications, revisits, and OASIS discharges
• Complete accurate and timely clinical documentation using electronic medical record systems
• Meet productivity requirements consistent with a full-time home health caseload
• Participate in on-call and phone triage rotation, covering evening and overnight hours on a rotating basis (approximately one to two weeks per month); occasional home visits may be required
• Deliver compassionate, patient-centered care with a strong focus on customer service excellence and professional conduct
• Manage time effectively to maintain an independent caseload while adhering to visit schedules

Required Qualifications

• Active and unrestricted Registered Nurse (RN) license in hand at time of submission
• Minimum of 2 years of home health nursing experience
• Demonstrated experience with Homecare HomeBase and OASIS documentation within the last year — required for consideration
• Experience with on-call and phone triage responsibilities
• Reliable, dependable, and possessing excellent time management skills
• Strong interpersonal skills with a commitment to professionalism, compassion, and customer service excellence

Preferred Experience / Skills

• Experience with WellSky EMR
• Wound care experience
