Contract (13 Weeks) | Full-Time | Onsite | Day Shift, Monday–Friday (makeup visits on weekends as needed)

Key Responsibilities

• Provide outpatient physical therapy services, with approximately 20% of visits conducted at assisted living facilities
• Manage a caseload ratio of 6–7 patients per day with an expected weekly mileage range of 50–75 miles
• Attend daily stand-up meetings at 8:30 AM, Monday through Friday
• Document patient care accurately using Wellsky charting software
• Adhere to professional dress standards including clean scrubs and closed-toe shoes

Required Qualifications

• Active Physical Therapist license with a minimum of 2 years of recent licensed PT experience
• At least 1 recently completed travel assignment required; new graduates will not be considered
• Experience with home care and HomeCare HomeBase (Homebase) required
• Current BLS and First Aid certifications required
• COVID-19 vaccination and booster(s) required per state guidelines; medical/religious exemptions considered on a case-by-case basis with required weekly testing if approved

Preferred Experience / Skills

• Experience working in outpatient and assisted living or similar community-based settings
• Familiarity with Wellsky electronic medical records platform
