Contract | Full-Time | Days (8:00 AM – 4:30 PM) | 40 Hours per Week | Onsite
Key Responsibilities
- Provide skilled hospice nursing care to patients in a home health setting
- Conduct patient assessments and develop individualized care plans in alignment with hospice goals
- Coordinate with interdisciplinary team members to ensure continuity and quality of end-of-life care
- Document patient care activities and clinical notes accurately using electronic health record systems
- Communicate with patients, families, and caregivers to provide education and emotional support
Required Qualifications
- Active Registered Nurse (RN) license in good standing
- Prior hospice nursing experience required
- Prior home health nursing experience required
- Proficiency with HCHB (Homecare Homebase) electronic health record system required
Preferred Experience / Skills
- Comfort and flexibility with optional weekend availability
- Willingness to participate in voluntary on-call rotation
