Key Responsibilities

• Perform and document comprehensive patient assessments, incorporating culture, age, developmental level, and risk factors in accordance with RN scope of practice and hospital policies.
• Develop individualized care plans, determine desired outcomes in coordination with patients and families, and implement patient care therapies and procedures with clinical autonomy.
• Recognize, respond to, and communicate changes in patient condition; conduct ongoing assessments and document responses to procedures and therapies using the nursing process.
• Delegate tasks appropriately to team members and monitor accuracy and completeness of delegated patient care activities.
• Facilitate patient discharge processes including timely execution of discharge orders, discharge teaching, and coordination with Case Management.
• Actively participate in unit-based quality improvement activities including patient satisfaction, fall prevention, pressure ulcer prevention, CAUTI reduction, and documentation auditing.

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the applicable state.
• Basic Life Support (BLS) certification required.
• Associate Degree in Nursing (ADN or ASN) required.
• Ability to communicate and collaborate effectively with interdisciplinary care team members.
• Proficiency with or ability to be trained on computer systems and clinical applications.

Preferred Experience / Skills

• Previous clinical nursing experience in a hospital or acute care setting.
• Bachelor of Science in Nursing (BSN).
