Contract | Days | Full-Time | Onsite | 5 days per week, approximately 8 hours per day | Expected 36 hours per week

Key Responsibilities

• Provide physical therapy services across Skilled Nursing Facility (SNF) and Home Health (HH) settings in an approximately 50/50 split, subject to fluctuation
• Supervise 2 Physical Therapist Assistants (PTAs) in the delivery of patient care
• Meet productivity expectations of 80% in the SNF setting and approximately 6 patients per day in the Home Health setting
• Document patient care using Net Health (SNF) and HCHB (Home Health) software platforms
• Provide occasional coverage at SNF or Home Health locations in nearby areas as needed

Required Qualifications

• Active Physical Therapist (PT) license in the state of Nebraska or eligibility to obtain licensure
• Ability to work across both Skilled Nursing Facility and Home Health care settings
• Proficiency with or ability to learn electronic documentation systems
• Must live at least 80 miles from the facility to be eligible for standard contract rates

Preferred Experience / Skills

• Prior experience in SNF and/or Home Health settings
• Experience supervising PTAs or other therapy support staff
• Familiarity with Net Health and/or HCHB documentation platforms
