Key Responsibilities

• Provide home health physical therapy services to patients across the Baytown and surrounding areas region
• Complete required visit points each week (32–34 points) in accordance with scheduling guidelines, with drive time factored into scheduling
• Document all patient visits and clinical notes accurately using Wellsky charting software
• Call in to report all missed visits and changes in visit frequency or inability to contact patients
• Maintain professional appearance including clean scrubs, closed-toe shoes, and minimal facial jewelry (small nose stud and earrings permitted)
• Adhere to a 1-point-per-visit structure; overtime is compensated when drive time pushes the workweek beyond 40 hours

Required Qualifications

• Active Physical Therapist (PT) license in the state of Texas
• Minimum of 2 years of recent licensed PT experience required (private duty experience does not qualify; new graduates are not eligible)
• Home health experience required — first travel assignment considered if home health background is present
• BLS and First Aid certifications required
• Proficiency or ability to chart in Wellsky
• Candidates are encouraged to secure housing arrangements prior to submission

Preferred Experience / Skills

• Prior travel therapy assignment experience
• Familiarity with GSA mileage reimbursement and paid drive time structures
• Experience managing a high-volume visit schedule in a home health setting
