Key Responsibilities

• Perform and document comprehensive patient assessments, incorporating culture, age, developmental level, risk factors, and presenting complaint in accordance with RN scope of practice and facility policies
• Develop individualized care plans, determine desired outcomes in coordination with patients and families, and fully implement patient care therapies and procedures within scope of practice
• Recognize, respond to, and communicate changes in patient condition; conduct ongoing assessments and document responses to procedures and therapies using the nursing process
• Facilitate admission, transfer, and discharge activities including timely execution of discharge orders, discharge teaching, and coordination with Case Management
• Delegate tasks appropriately and monitor accuracy and completeness of delegated patient care provided by other team members
• Actively participate in unit-based quality improvement activities including patient satisfaction, fall prevention, pressure ulcer prevention, CAUTI reduction, and documentation auditing

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of Kentucky
• Basic Life Support (BLS) certification required
• Associate Degree in Nursing (ADN) or Associate of Science in Nursing (ASN)
• Ability to communicate and collaborate effectively with interdisciplinary care team members
• Proficiency with or ability to be trained on relevant computer systems and clinical applications

Preferred Experience / Skills

• Previous clinical nursing experience in a medical surgical or telemetry setting
• Bachelor of Science in Nursing (BSN)
• Experience with evidence-based practice and quality improvement initiatives
