Key Responsibilities

• Provide timely communication to physicians, families, and other stakeholders regarding significant changes in resident condition
• Complete all applicable resident assessments and maintain accurate, thorough resident records
• Administer medications in accordance with the 5 Rights of Medication Administration
• Perform wound care and wound assessments as needed
• Monitor residents for changes in condition and act as a floor leader by assessing and directing situations as needed
• Report any allegations of abuse, neglect, or misappropriation to appropriate facility leadership

Required Qualifications

• Minimum two-year degree in nursing required
• Must hold and maintain an active Wisconsin Registered Nurse (RN) license
• Current CPR/BLS certification required
• Exceptional accuracy and attention to detail

Preferred Experience / Skills

• Four-year degree in nursing preferred
• 1+ years of related nursing experience, preferably in a long-term care or skilled nursing setting
