Schedule / Job Type
Contract | Days | Monday through Friday, 7:00 AM–3:00 PM or 8:00 AM–4:00 PM (flexible) | Expected 32 hours/week
Key Responsibilities
- Provide occupational therapy services to patients in both Skilled Nursing Facility (SNF) and Home Health settings
- Meet productivity expectations of 87% in the SNF setting and 95% in the Home Health setting
- Utilize Net Health and Home Care Home Base software for documentation and patient management
- Deliver patient care under the supervision of an on-site Therapy Supervisor
- Follow established care plans and contribute to patient progress toward functional goals
Required Qualifications
- Active COTA licensure or certification in the state of Minnesota (or eligibility to obtain)
- Must reside within 80 miles of the facility to be eligible for this position
- Ability to work across SNF and Home Health care settings
- Proficiency or ability to learn electronic documentation platforms
Preferred Experience / Skills
- New graduates are welcome to apply
- Prior experience in SNF or Home Health settings is a plus
- Familiarity with Net Health and/or Home Care Home Base documentation software
