Key Responsibilities
- Provide individualized, goal-oriented nursing care to patients in their homes using the nursing process in accordance with physician orders
- Manage an active patient caseload, serving as the lead clinical contact for assigned cases
- Function as Case Manager for cases in which you are the lead discipline
- Mentor and support a Nurse Resident, helping them build clinical confidence and skills in a home health setting
Required Qualifications
- Active Registered Nurse (RN) license in the state of Vermont
- Experience in home health or a related clinical nursing setting
- Ability to manage an independent patient caseload with minimal supervision
- Strong clinical assessment and care planning skills
Preferred Experience / Skills
- Prior experience in a Case Manager or lead nursing role
- Experience mentoring or precepting new graduate nurses or nurse residents
- Background in hospice or community-based care
