Travel - Licensed Master Social Worker (LMSW) / Social Worker - Case Management

Contract | Full-Time | Days | Monday–Friday, 8:00 AM–4:30 PM | Rotating weekends (Saturday & Sunday, 8:00 AM–4:30 PM) | No call required
Key Responsibilities
- Conduct face-to-face interviews with patients and families to assess needs and coordinate discharge planning
- Perform InterQual reviews on all patients, including observation reviews, DPEs, and discharge planning activities
- Participate in multidisciplinary rounds (MDRs) on each unit daily
- Manage a patient caseload with a ratio of approximately 1:40 Monday–Friday
- Float throughout the entire facility, including Women & Children's Health and the Emergency Department, as needed
- Document and track case management activities using facility EMR systems
Required Qualifications
- Licensed Master Social Worker (LMSW) — active and unrestricted license required
- Case management experience required
- Proficiency with Midas and InterQual required
- Ability to work rotating weekends (Saturday & Sunday)
Preferred Experience / Skills
- Knowledge of local and regional community resources
- NATE certification or familiarity with NATE processes
- Experience in an acute care or hospital-based case management setting
- Outgoing, collaborative personality comfortable working across multiple units and with diverse teams
