Key Responsibilities
- Provide direct patient care in a long-term care/skilled nursing facility setting
- Utilize PointClickCare (PCC) electronic health records system for documentation and care coordination
- Adhere to all facility policies, including presenting valid state or federal issued photo identification upon entry
- Wear a visible photo badge at all times while on assignment
Required Qualifications
- Active and unrestricted LPN license in the state of Vermont
- Proficiency with PointClickCare (PCC) — training in this system is required
- Valid state or federal issued photo identification
- COVID-19 vaccination (original or updated series) or a documented religious/medical exemption on file with employer
- Local candidates only
Preferred Experience / Skills
- Prior experience in a long-term care or skilled nursing facility environment
- Familiarity with agency/registry staffing protocols and compliance requirements
