What You'll Be Doing

• Lead and manage the rehabilitation department within a welcoming 72-bed facility offering long-term, short-term, and memory care.

• Oversee clinical operations for an adult to geriatric patient population, ensuring high-quality medical and personal care services.

• Manage a standard 40-hour work week across 5 days, including participation in a weekend rotation.

• Coordinate therapy services for residents in a serene environment adjacent to the White Mountain National Forest.

• Ensure departmental compliance with Medicaid and Medicare regulations and documentation standards.

• Foster a supportive team environment focused on exceptional patient outcomes and functional improvement.

• Act as a key liaison between the rehab department and facility leadership to drive operational excellence.

What We're Looking For

• Licensure: Must hold a valid professional license in a therapy discipline (Physical Therapy, Occupational Therapy, or Speech-Language Pathology) in the state of New Hampshire.

• Experience: Proven leadership or management experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting.

• Clinical Knowledge: Strong understanding of geriatric rehabilitation, memory care protocols, and Medicare/Medicaid reimbursement models.

• Soft Skills: Exceptional organizational skills and a passion for leading clinical teams in a small, community-focused facility.

• Requirements: Ability to commit to a full-time permanent schedule with weekend rotation flexibility.
