Job Title: LVN / LPN - LTC/SNF

Schedule / Job Type: Full-time
Key Responsibilities:
• Provide direct patient care to residents in a long-term care facility, including administering medications, monitoring vital signs, and performing various medical procedures. • Develop and implement individualized care plans to meet the unique needs of each resident, including managing chronic conditions and preventing complications. • Collaborate with interdisciplinary teams to coordinate care and ensure seamless communication with physicians, therapists, and other healthcare professionals. • Maintain accurate and up-to-date resident records, including medical history, treatment plans, and progress notes. • Participate in ongoing education and training to stay current on best practices and industry developments.
Required Qualifications:
• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of NM. • Ability to obtain certification in medication administration, if required by the facility. • Basic Life Support (BLS) certification. • Strong communication and interpersonal skills.
Preferred Experience / Skills:
• Experience working in long-term care or skilled nursing facilities. • Knowledge of medication administration and management. • Ability to lift up to 50 pounds and move residents as needed.
