Key Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility.
• Develop and implement personalized treatment plans to support recovery and improve patient mobility.
• Monitor patient progress and adjust therapy plans accordingly.
• Collaborate with nursing staff and multidisciplinary teams to ensure optimal patient care.
• Document patient assessments, treatments, and progress accurately.
• Support both short-term rehab and long-term care patients in a compassionate manner.

Required Qualifications

• Valid state licensure as a Physical Therapist.
• Strong knowledge of therapeutic techniques and patient care standards.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Prior experience in a skilled nursing facility is preferred.

Preferred Experience / Skills

• Experience working with post-hospital rehab patients.
• Familiarity with documentation and compliance standards in healthcare.
