Key Responsibilities
• Assist licensed therapists in providing rehabilitation services to patients in a skilled nursing facility.
• Implement treatment plans and monitor patient progress under supervision.
• Support patient recovery and mobility through therapeutic exercises and activities.
• Maintain accurate documentation of patient care and progress reports.
• Collaborate with healthcare team to ensure optimal patient outcomes.
• Contribute to a positive and supportive environment for patients and staff.

Required Qualifications
• Valid state license or certification as a Physical Therapy Assistant.
• Prior experience working in a skilled nursing or long-term care setting preferred.
• Strong understanding of rehabilitation procedures and patient care techniques.
• Excellent communication and patient interaction skills.
• Ability to work effectively as part of a multidisciplinary team.
• Detail-oriented with strong organizational skills.

Preferred Experience / Skills
• Experience working with elderly or rehab patients.
• Knowledge of documentation and electronic health records.
• Bilingual abilities a plus.
