Key Responsibilities

• Provide physical therapy assessments and treatments to patients in a skilled nursing facility
• Develop and implement personalized rehabilitation plans focused on patient recovery and mobility
• Collaborate with an interdisciplinary team to ensure comprehensive patient care
• Maintain accurate documentation of therapies and patient progress
• Assist in short-term rehab and long-term care services within a 180-bed facility

Required Qualifications

• Valid and active Physical Therapist license in the state of Pennsylvania
• Proven experience in a skilled nursing or long-term care setting
• Strong understanding of rehabilitation techniques and patient needs
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team

Preferred Experience / Skills

• Experience working in a facility offering both short-term rehabilitation and long-term care
• Knowledge of relevant healthcare regulations and compliance standards
• Ability to work one weekend day per month

Additional Notes

• Compensation includes a 10% salary placement fee
• Salary range: $40 - $52.40 per hour
• Shifts are 8.5 hours per day, Monday through Friday, with one weekend day per month required
