Job Title

• Registered Nurse - LTAC

Schedule / Job Type

• Full-time, night shifts, 8-hour shifts, on-site

Key Responsibilities

• Provide direct nursing care to residents in an LTAC setting, ensuring timely and accurate assessments.
• Communicate significant changes in resident conditions to physicians and families promptly.
• Administer medications following the 5 Rights and perform wound care as needed.
• Assist Certified Nursing Assistants with resident needs and respond to call lights.
• Monitor residents for changes in condition and act as a floor leader to assess and manage situations.
• Maintain detailed and accurate resident records and documentation.
• Report any allegations of abuse, neglect, or misappropriation to supervisory staff.
• Perform additional duties as assigned to support resident care and facility operations.

Required Qualifications

• Two-year degree in nursing (associate degree) required; four-year degree preferred.
• Valid Wisconsin Registered Nursing license and ongoing licensure maintenance.
• Current CPR/BLS certification.
• Exceptional attention to detail and accuracy in clinical documentation.
• 1+ years of nursing experience in a healthcare setting preferred.

Preferred Experience / Skills

• Experience working in long-term acute care (LTAC) or similar environments.
• Strong communication skills and ability to work effectively in a team.
