Key Responsibilities

• Provide physical therapy services to patients in Skilled Nursing Facilities (SNF) and Assisted Living/Independent Living (AL/IL) communities.
• Evaluate patient conditions and develop individualized treatment plans.
• Implement therapeutic interventions to improve mobility, strength, and overall function.
• Document patient progress and communicate with healthcare team members.
• Ensure compliance with relevant healthcare regulations and safety standards.

Required Qualifications

• Valid state licensure as a Physical Therapist.
• Proven experience working in SNF and assisted living facilities.
• Strong knowledge of physical therapy principles and practices.
• Excellent communication and patient care skills.

Preferred Experience / Skills

• Previous experience in long-term care settings.
• Ability to work efficiently in a fast-paced environment.
• Bilingual skills are a plus.
