Key Responsibilities
- Provide physical therapy services to patients in a skilled nursing facility (SNF) setting
- Develop and implement individualized treatment plans based on patient needs
- Document patient progress and communicate updates to healthcare teams
- Ensure compliance with all regulatory and safety standards
- Collaborate with interdisciplinary teams to optimize patient care
Required Qualifications
- Minimum of 1 year of professional SNF (Skilled Nursing Facility) experience
- Valid state license as a Physical Therapist
- Strong understanding of patient assessment and treatment planning in a SNF environment
- Excellent communication and documentation skills
Preferred Experience / Skills
- Pediatric or geriatric therapy experience (preferred but not required)
- Bilingual abilities or additional certifications a plus
