Key Responsibilities

• Provide outpatient physical therapy services to clients within a clinic setting, with some visits in assisted living facilities
• Attend daily stand-up meetings at 8:30 AM Monday through Friday
• Document patient progress using Wellsky charting system
• Travel within a 50-mile radius of the facility, covering an estimated 50-75 miles weekly
• Maintain professional appearance with clean scrubs and closed-toe shoes
• Ensure adherence to facility protocols and patient care standards

Required Qualifications

• Valid Physical Therapist license with at least 2 years of recent licensed PT experience
• Recent completion of at least one travel assignment in homecare or home-based settings
• BLS and First Aid certifications
• Willingness to work a 13-week contract with guaranteed 36 or 40 hours weekly

Preferred Experience / Skills

• Previous experience working with outpatient populations
• Familiarity with Wellsky or similar charting systems
• Ability to work independently while participating in team meetings
• Experience with homecare visits and managing travel logistics
