Job Title

Schedule / Job Type
• Days shift, part-time, split position, approximately 35 hours per week

Key Responsibilities
• Provide skilled physical therapy services to patients in a skilled nursing facility (SNF) setting
• Perform assessments and develop treatment plans tailored to individual patient needs
• Document patient progress and communicate with healthcare team members
• Ensure compliance with facility protocols and legal requirements
• Maintain accurate and timely documentation of treatments and outcomes

Required Qualifications
• Minimum of 1 year of professional SNF (Skilled Nursing Facility) experience
• CT fingerprints required for licensing/clearance
• Valid state license or eligibility to practice as a Physical Therapist in Connecticut

Preferred Experience / Skills
• Proven track record of achieving high productivity, with at least 88% after 2 weeks
• Ability to work in a split position environment
• Strong communication and teamwork skills
