Key Responsibilities

• Provide nursing care to residents in a long-term care or skilled nursing facility setting
• Assist patients with all personal cares, transfers, toileting, and activities of daily living (ADLs)
• Manage common diagnoses such as CHF, HF, falls, hypertension, and diabetes
• Support patient safety and comfort by adhering to care plans and protocols
• Collaborate with healthcare team to ensure quality patient care

Required Qualifications

• Valid LVN or LPN license in the state of Minnesota
• Experience working in LTC or SNF environments preferred
  -Ability to manage multiple patients and prioritize tasks effectively
• Basic knowledge of patient care procedures and safety protocols

Preferred Experience / Skills

• Familiarity with common chronic conditions such as CHF, HF, and diabetic management
• Experience with patient transfers and assisting with ADLs
• Ability to work in a team-oriented environment
