Job Title

Schedule / Job Type
• Day shift, 5 days per week

Key Responsibilities
• Provide skilled physical therapy services to patients in a home health setting.
• Proficiently perform and document assessments using OASIS for all assigned cases.
• Travel to patient homes, reimbursed at IRS mileage rates, ensuring compliance with travel policies.
• Manage patient care in accordance with facility protocols, including acute care requirements.
• Maintain all necessary certifications and licenses, including VA licenses or proof of compact license.
• Complete assigned charting using electronic medical records, specifically EPIC when required.

Required Qualifications
• Minimum of one year of home health experience (post-graduation).
• Valid state licensure as a Physical Therapist; VA license or proof of compact license required at submission.
• Proficiency in OASIS documentation.
• Certifications: BLS, ACLS, and PALS through the American Heart Association (AHA).
• Experience with EPIC charting in acute care hospitals under Riverside Health is preferred.
• Prior NIH and stroke education for RN roles within Riverside acute care facilities.

Preferred Experience / Skills
• Recent specialty experience within the last year in a similar setting.
• Strong understanding of Riverside’s electronic charting systems and protocols.
• Ability to work independently and adapt to floating needs as required by the facility.

Additional Notes
• Local candidates are accepted at the same base rate.
• Candidates must have at least one supervisory reference for submission (two required for compliance).
• Travel and internship experience during education does not count toward experience.
• No call, and minimal RTO requested; all RTO must be communicated upfront.
• Floating assignments may be requested based on facility needs.
• Candidates cannot work both Riverside travel and per diem contracts simultaneously; choice required.
