Key Responsibilities

• Provide home health and hospice nursing care to patients, including assessments, wound care, IV therapy, and management of drains and devices.
• Document patient progress using proficiency in OASIS and ensure compliance with infection control practices.
• Participate in patient admissions, change of condition reviews, recertifications, and resumption of care activities.
• Collaborate with an interdisciplinary team, including CNAs, RN coordinators, and supervisors, to deliver quality patient care.
• Participate in on-call rotations for nights, weekends, and holidays as scheduled.
• Follow agency dress code and clock in/out at the facility as required.

Required Qualifications

• Valid Registered Nurse (RN) license.
• Recent home health experience within the last 2 years.
• Proficiency in OASIS documentation.
• Experience with wound care and infection control practices.
• BLS certification.
• Ability to see approximately 6 patients per day, including admissions, re-certifications, and on-call duties.

Preferred Experience / Skills

• Experience managing complex patients such as those with CHF, Diabetes, Renal Disease.
• Ability to work independently in a home care setting.
• Strong communication and documentation skills.
