Job Title

Schedule / Job Type
• Full-time, Day shift, 5 days per week

Key Responsibilities
• Provide outpatient physical therapy services in a critical access hospital setting
• Assess and develop plan of care, discharge plans, and evaluate patient progress
• Document patient assessments, goals, and evaluations in EMR system
• Manage patient treatment for geriatrics and orthopedics populations
• Utilize equipment such as gait belts, wheelchairs, prosthetics, and patient lifts
• Collaborate with multidisciplinary team to meet patient care goals

Required Qualifications
• Valid state license as a Physical Therapist
• Minimum of 1 year of recent outpatient or acute care experience in specialty therapy
• Current BLS certification (preferred)
• Ability to work 8-hour day shift from 8:00 AM to 4:30 PM
• Fluency in documentation including assessment, evaluation, plan of care, and goal setting

Preferred Experience / Skills
• 2 or more years of relevant clinical experience strongly preferred
• Experience with outpatient, inpatient, or skilled nursing facilities
• Familiarity with EPIC electronic medical record system
• Ability to work within a flexible scope, including floating between departments and facilities
