Key Responsibilities

• Provide direct nursing care to residents in a long-term care / skilled nursing facility setting
• Manage and monitor residents with common diagnoses including hypertension, diabetes, and those requiring assistance with ADLs and transfers
• Administer medications and treatments in accordance with physician orders and facility protocols
• Document resident care and clinical observations using the facility's electronic medical records system (PCC)
• Collaborate with medication aides and interdisciplinary care team members to ensure resident safety and quality of care
• Adhere to weekend and holiday rotation requirements as scheduled

Required Qualifications

• Active and unrestricted LPN or LVN license in the state of Iowa
• Experience in a long-term care or skilled nursing facility (LTC/SNF) setting
• Ability to work every other weekend as required
• Ability to participate in holiday rotation on an alternating annual basis
• Proficiency or ability to learn Point of Care (PCC) electronic medical records system

Preferred Experience / Skills

• Prior experience working with residents requiring ADL assistance and transfers
• Familiarity with PCC (PointClickCare) EMR platform
• Experience managing residents with hypertension and diabetes in a skilled nursing environment
