Key Responsibilities
- Provide direct nursing care to residents in a long-term care / skilled nursing facility setting
- Manage and monitor residents with common diagnoses including hypertension, diabetes, and those requiring assistance with ADLs and transfers
- Administer medications and treatments in accordance with physician orders and facility protocols
- Document resident care and clinical observations using the facility's electronic medical records system (PCC)
- Collaborate with medication aides and interdisciplinary care team members to ensure resident safety and quality of care
- Adhere to weekend and holiday rotation requirements as scheduled
Required Qualifications
- Active and unrestricted LPN or LVN license in the state of Iowa
- Experience in a long-term care or skilled nursing facility (LTC/SNF) setting
- Ability to work every other weekend as required
- Ability to participate in holiday rotation on an alternating annual basis
- Proficiency or ability to learn Point of Care (PCC) electronic medical records system
Preferred Experience / Skills
- Prior experience working with residents requiring ADL assistance and transfers
- Familiarity with PCC (PointClickCare) EMR platform
- Experience managing residents with hypertension and diabetes in a skilled nursing environment
