Schedule / Job Type
Full-time, dayshift, 8-hour shifts, Monday through Friday

Key Responsibilities

• Provide high-quality nursing care to residents within a long-term care or skilled nursing facility.
• Monitor patient conditions and administer medications as prescribed.
• Collaborate with healthcare team to develop and implement care plans.
• Document patient progress and care details using electronic medical records.
• Assist with patient hygiene, mobility, and daily living activities.
• Ensure compliance with facility policies and safety standards.

Required Qualifications

• Valid LPN/LVN license in the state of Minnesota.
• Proven experience in long-term care or skilled nursing facilities preferred.
• Ability to work independently and as part of a healthcare team.
• Strong clinical and documentation skills.
• Basic proficiency with electronic medical records (EMR), preferably Matrix.

Preferred Experience / Skills

• Experience providing care to general LTC populations and short-term residents.
• Ability to work at least every other weekend and during holidays (half-day).
• Flexibility to work both 8-hour and 12-hour shifts.
