Job Title

Schedule / Job Type

• Full-time, Night shifts, primarily 8-hour shifts, with some 12-hour shifts available

Key Responsibilities

• Provide patient care in a Skilled Nursing Facility focusing on dementia.
• Adhere to facility protocols and documentation procedures such as EMR and paper flowsheets.
• Collaborate with healthcare team to ensure quality resident care.
• Participate in internal team shifts every other weekend and holiday.
• Maintain compliance with state licensing and facility requirements.

Required Qualifications

• Valid and active Minnesota RN license.
• Active MN SNSA registration.
• Ability to work night shifts and adapt to shift rotations.

Preferred Experience / Skills

• Experience with dementia patient care.
• Knowledge of electronic medical records systems such as Matrix or paper flowsheets.
• Ability to work effectively in a dynamic, multi-story facility environment.
