Key Responsibilities
• Provide occupational therapy services to patients in a home health setting within designated territories.
• Conduct assessments, develop treatment plans, and deliver interventions to improve patient functional abilities.
• Document patient progress and coordinate care with healthcare teams and families.
• Travel within assigned counties to patient homes, possibly outside the area or during weekends, depending on the assignment.
• Comply with all licensing, certification, and documentation requirements for home health care.
• Adhere to agency and client policies, including scheduling, time tracking, and mandatory disclosures.

Required Qualifications
• Current California Occupational Therapist License
• Valid BLS certification
• Valid driver’s license, auto insurance, and vehicle registration
• Up-to-date TB, MMR, Varicella, HepB, TDaP, and annual Flu vaccinations
• Successful completion of drug screening and physical assessment

Preferred Experience / Skills
• Home health experience preferred but not required.
• Ability to travel within the service area and work flexible hours, including weekends if needed.
• Strong documentation skills and ability to follow care protocols.
