Key Responsibilities
- Provide skilled nursing care and support to residents in a long-term care or skilled nursing facility
- Monitor and record patient health status, report significant changes to the healthcare team
- Administer medications and treatments following prescribed orders
- Assist with activities of daily living and ensure resident comfort and safety
- Collaborate with interdisciplinary team to develop and implement care plans
Required Qualifications
- Active Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license
- Previous experience in a Long-Term Care (LTC) or Skilled Nursing Facility (SNF) setting preferred
- Knowledge of medical terminology, safety procedures, and medication administration
- Strong communication and interpersonal skills
Preferred Experience / Skills
- Prior experience working 12-hour shifts or in a similar fast-paced healthcare environment
- Certification in CPR and First Aid
- Ability to work independently and as part of a team
