- Full-time, Mids (Flexible shifts, 8.5 hours per shift)
Key Responsibilities
- Provide patient care and support in long-term care or skilled nursing facilities (LTC/SNF)
- Administer medications and treatments as prescribed by physicians
- Monitor patient conditions and report changes to the healthcare team
- Document patient care activities accurately and timely
- Collaborate with interdisciplinary teams to ensure quality patient outcomes
Required Qualifications
- Valid LPN/LVN license in the state of Pennsylvania
- Prior experience in long-term care or skilled nursing facilities preferred
- Ability to work 8.5-hour shifts, including mids (midday shifts)
- Strong communication and patient care skills
- Knowledge of medical terminology and healthcare protocols
Preferred Experience / Skills
- Experience working with elderly or chronically ill patients
- Certifications such as CPR/BLS
