Key Responsibilities

• Provide outpatient physical therapy services to a diverse patient population.
• Assess patient conditions and develop individualized treatment plans.
• Implement therapeutic interventions to improve mobility, strength, and function.
• Document patient progress and maintain accurate records in electronic health systems.
• Collaborate with healthcare team members to ensure comprehensive patient care.

Required Qualifications

• Valid Physical Therapist license for the state of Oregon.
• Basic Life Support (BLS) certification.
• Own reliable transportation for travel within the service area.
• Experience with EPIC/Oasis charting systems.

Preferred Experience / Skills

• Previous home health or outpatient therapy experience.
• Ability to adapt to a fast-paced environment and manage a varied caseload.
• Strong communication and patient education skills.
