Key Responsibilities

• Provide acute care therapy services within a Level II Trauma Center.
• Collaborate on discharge planning and care coordination for diverse patient populations.
• Float between hospital units, including multiple campuses, providing consistent patient care.
• Document patient progress and treatment in EPIC charting system accurately.
• Work approximately 72 hours every two weeks, including working every other weekend and three major holidays.
• Adhere to hospital policies, including maintaining required certifications and licensure.

Required Qualifications

• Valid WA State license for MSW (Master of Social Work).
• Current BLS certification.
• Minimum of 2 years of acute care experience, including discharge planning and care coordination.
• Strong preference for candidates with diverse Case Management experience across Med Surg, LLOS, and EDC units.
• Ability to float within hospital units and to nearby facilities as needed.
• Experience with EPIC charting system.

Preferred Experience / Skills

• Experience working in a Level II trauma setting.
• Previous experience with discharge planning and multi-unit case management.
• Flexibility to work with holiday schedules and on-call requirements.
