- Full-time, Contract (13 weeks), Day shifts, 4 days per week
Key Responsibilities
- Provide discharge planning and care coordination for patients in a hospital setting
- Work across multiple units, including Med Surg, LLOS, and EDC, providing diverse case management support
- Collaborate with interdisciplinary teams to develop and implement patient care plans
- Document all patient interactions and plan updates accurately within EPIC electronic health record system
- float between hospital units as needed to ensure comprehensive patient care coverage
- Assist in compliance with hospital policies and state licensure requirements
Required Qualifications
- Valid Washington State Licensure as a Social Worker or relevant professional license
- BLS certification required
- Minimum of 1 year of acute care experience, with strong discharge planning or care coordination background
- Ability to work every other weekend and holiday shifts (Thanksgiving, Christmas Eve, Christmas Day, New Year’s Eve, New Year’s Day)
- Experience working with diverse patient populations and complex cases
Preferred Experience / Skills
- Experience in multiple case management roles within hospital settings such as Med Surg, LLOS, or EDC
- Familiarity with EPIC electronic health record system
- Ability to float across units and campuses efficiently
- Prior experience in a Level II Trauma Center is a plus
