• Full-time, day shifts, 5 days per week

Key Responsibilities

• Provide skilled nursing care to home health patients in accordance with physician orders and agency policies
• Conduct OASIS assessments; experience preferred, but not required
• Administer treatments, monitor patient progress, and educate patients and families about health management
• Collaborate with interdisciplinary teams to develop and update care plans
• Ensure compliance with all documentation and regulatory requirements, including timely submission of required forms
• Follow all safety and infection control protocols during patient visits

Required Qualifications

• Valid RN license (pending licenses accepted; must obtain prior to start)
• BLS certification required
• At least 1-2 years of recent home health or related experience preferred
• Willingness to float within scope as needed to meet facility requirements

Preferred Experience / Skills

• Knowledge of EPIC documentation system (preferred, but not mandatory)
• Experience with cardiac dysrhythmia assessments and moderate sedation testing (training provided on first day of orientation)
• Strong assessment, communication, and patient education skills
