LVN / LPN LTC/SNF

Job Title

• Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

• Franklin, NH

Schedule / Job Type

• Full-time, night shifts, 3 shifts per week, approximately 12.5 hours per shift

Key Responsibilities

• Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting
• Monitor patient health status and report changes to appropriate healthcare team members
• Administer medications and treatments as prescribed
• Document patient care activities accurately and promptly
• Collaborate with interdisciplinary team to ensure quality resident care
• Work every other weekend as part of the scheduling requirement

Required Qualifications

• Valid LVN or LPN license in the state of New Hampshire
• Experience working in LTC (Long-Term Care) or SNF (Skilled Nursing Facility) settings preferred
• Ability to work night shifts and adapt to a flexible schedule

Preferred Experience / Skills

• Prior experience providing care in a similar clinical environment
• Strong communication and teamwork skills
• Knowledge of state and federal healthcare regulations
