Key Responsibilities
- Provide direct patient care in a long-term care or skilled nursing facility setting
- Monitor patient health and record vital signs and medical information
- Collaborate with healthcare teams to develop and implement patient care plans
- Ensure compliance with healthcare regulations and facility policies
- Educate patients and family members on health management and post-discharge care
Required Qualifications
- Valid nursing license (RN) in the state of New Hampshire
- Proven experience in long-term care or skilled nursing facilities preferred
- Strong patient assessment and documentation skills
- Excellent communication and teamwork abilities
Preferred Experience / Skills
- Previous experience working in a SNF or LTC environment
- BLS and CPR certification
- Familiarity with electronic health records (EHR) systems
