Contract | Days | 10-hour shifts, 4 days per week | Rotated on-call night coverage 1–2 times per week | Every 3rd weekend | 32 guaranteed hours per week
Key Responsibilities
- Provide hospice and palliative care to assigned clients in a safe, compassionate, and efficient manner in accordance with individualized plans of care.
- Perform initial and ongoing client assessments, participate in the creation and implementation of the interdisciplinary plan of care, and coordinate services to ensure continuity of care.
- Recognize and respond to diverse cultural attitudes toward death and dying, and support clients and families through the grieving process.
- Process physician orders, complete required documentation within appropriate timeframes, and conduct quality assurance audits as assigned.
- Collaborate with the interdisciplinary team, participate in team meetings, in-services, and educational opportunities.
- Explain advance directives, clinical requirements, and available interdisciplinary services to clients and families.
Required Qualifications
- Graduate of an accredited school of nursing.
- Active Registered Nurse (RN) licensure valid for the state of practice.
- Current CPR certification through the American Heart Association (AHA).
- Ability to work independently with minimal supervision while seeking guidance as appropriate.
- Valid driver's license and reliable personal vehicle for travel to patient homes and care facilities; mileage reimbursement is provided.
Preferred Experience / Skills
- Prior experience in hospice or palliative care nursing.
- Familiarity with electronic medical records (EMR) systems for documentation and mileage tracking.
- Experience conducting admissions, death pronouncements, and supervisory visits.
