Key Responsibilities
- Provide direct physical therapy services to patients in an outpatient setting
- Evaluate, plan, and implement individualized treatment programs based on patient needs
- Complete required clinical documentation accurately and in a timely manner
- Collaborate with the care team to support patient progress and outcomes
- Maintain compliance with credentialing and facility-specific requirements throughout the assignment
Required Qualifications
- Active Physical Therapist license in the state of Oregon (permanent license required)
- Prior experience in an outpatient physical therapy setting
- Ability to meet contract credentialing and quality assurance requirements
- Willingness to complete any facility-specific onboarding documentation prior to assignment start
Preferred Experience / Skills
- Previous experience working in a high-volume outpatient rehabilitation environment
- Familiarity with travel or contract therapy assignments
