Long Term Care Day Nurse – LVN / LPN

Contract | Full-Time | Onsite | Day Shift (6:00 AM – 6:00 PM)

Key Responsibilities

• Administer medications and treatments as ordered by physicians to residents in a long-term care intermediate facility
• Assign, direct, and supervise CNA staff performing resident care, ensuring all tasks are completed appropriately
• Monitor and document resident status, including acute illness, physician visits, and changes in condition via timely nurse's notes
• Report changes in resident condition or personnel matters to nursing leadership in a timely manner
• Keep physicians and families informed of residents' status as needed, including notification of updates, changes, and new orders
• Complete all required documentation accurately and in a timely manner, and attend required meetings and educational sessions

Required Qualifications

• Active LVN or LPN license in good standing
• Experience in a long-term care or skilled nursing facility (LTC/SNF) setting
• Ability to administer medications and perform treatments as ordered
• Strong documentation skills with attention to accuracy and timeliness
• Ability to supervise and direct CNA staff effectively

Preferred Experience / Skills

• Prior experience as a charge nurse or in a supervisory nursing role
• Familiarity with memory care or dementia patient populations
• Experience serving as a treatment nurse in a long-term care environment
