Hospice Case Manager – Registered Nurse

Contract | Full-Time | Days | Monday – Friday | Onsite / Field-Based

Key Responsibilities

  • Manage a caseload of hospice patients receiving care in a home setting
  • Conduct patient assessments and develop individualized plans of care
  • Coordinate and communicate with interdisciplinary care teams to ensure quality patient outcomes
  • Provide skilled nursing care and symptom management consistent with hospice standards
  • Travel to patient homes throughout the service area, including rural locations
  • Document patient care accurately and in a timely manner using electronic medical records

Required Qualifications

  • Active Registered Nurse (RN) license in the state of Vermont or compact eligibility
  • Prior experience in hospice case management
  • Demonstrated experience delivering hospice care in a home-based setting
  • Ability to travel to patient homes throughout the assigned service area

Preferred Experience / Skills

  • Prior experience with rural travel in a home care or field-based nursing role
  • Proficiency with McKesson or Netsmart EMR platforms
