Key Responsibilities
- Provide skilled home health nursing care to patients in the West Portland, OR area and surrounding communities
- Visit up to 6 patients per day, meeting a standard productivity expectation of 30 points per day
- Complete clinical documentation using EPIC Remote Client for Home Health (laptop) and Rover (phone app), including OASIS assessments
- Manage daily patient visit schedule, with occasional travel radius of up to approximately 60 miles per day
- Coordinate care and communicate patient status in accordance with home health clinical standards
Required Qualifications
- Active Oregon Registered Nurse (RN) license
- Minimum 1 year of skilled home health nursing experience
- Current Basic Cardiac Life Support (BCLS) certification
- OASIS documentation experience required
- Reliable personal transportation required
- COVID-19 primary vaccination series plus at least one booster required
Preferred Experience / Skills
- Familiarity with EPIC Remote Client and/or Rover application for home health charting
- Experience meeting productivity point expectations in a home health setting
- Comfort navigating a wide geographic service area with efficient time management
