Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based

Key Responsibilities

• Provide skilled physical therapy assistant services to patients in a home health setting across the west Portland area and surrounding communities.
• Conduct patient visits in the field, with up to 6 patient visits per day and a standard productivity expectation of 30 points per day.
• Document patient care and progress using EPIC Remote Client (laptop) and Rover mobile application.
• Travel independently to patient homes within a service area; occasional daily drive radius of up to approximately 60 miles with up to 90 minutes of total drive time.
• Collaborate with the care team to deliver patient-centered treatment plans in accordance with home health standards.

Required Qualifications

• Active or pending Physical Therapy Assistant (PTA) license in the state of Oregon.
• Minimum of 1 year of skilled home health experience required.
• Basic Cardiac Life Support (BCLS) certification required.
• Reliable personal transportation required for daily patient visits.
• COVID-19 primary vaccination series plus one booster required.
• Experience with OASIS documentation in a home health setting.

Preferred Experience / Skills

• Familiarity with EPIC Remote Client and/or Rover charting platforms.
• Comfort working independently in a field-based, patient home environment.
• Experience meeting productivity point expectations in a home health setting.
