Contract | Onsite | Mid Shift (3:00 PM – 11:00 PM) | Every Other Weekend Required | Holiday Rotation May Apply

Key Responsibilities

• Administer medications and perform head-to-toe patient assessments
• Provide nursing care for residents with diagnoses including CHF, COPD, diabetes, dementia, and rehabilitation needs
• Manage IV therapy for fluids and antibiotics as needed
• Assume charge nurse duties as required
• Float throughout the facility as needed
• Participate in facility orientation including competency modules and skills check-offs

Required Qualifications

• Active LPN / LVN license
• Current BLS certification
• Minimum 1 year of experience in a long-term care or skilled nursing facility setting
• Comfort working in a locked dementia unit environment
• Ability to work every other weekend and potential holiday rotation

Preferred Experience / Skills

• Experience with Point Click Care (PCR) electronic medical records system
• Familiarity with SNF interdisciplinary team collaboration including PT/OT, dietary, social services, and case management
• Experience with BIPAP/CPAP patient care (no ventilated patients)
