Contract | Days, 5x8 Hours (8:00–17:00) | Full-Time | Onsite / Field-Based

Key Responsibilities

• Conduct home health visits including start of care OASIS assessments, post-surgical assessments, and follow-up visits across an assigned territory of approximately 40–80 miles
• Perform point-of-care services including wound care, wound VAC management, IV therapy (antibiotics, TPN), trach care, G-tube feedings, and incision assessment
• Complete OASIS documentation using Epic electronic health records system
• Operate clinical equipment including wound VAC, glucometer, pro-time/PT-INR machine, IV pumps, and central lines/ports
• Provide patient and family education on medications, wound care, and ADLs
• Participate in an assigned POD (group of zip codes) as part of a multidisciplinary team, with flexibility to assist other PODs as needed; participate in on-call rotation every 3rd to 4th weekend

Required Qualifications

• Active, unencumbered Registered Nurse (RN) license in Illinois (license in hand required at time of submittal) or applicable compact license matching permanent residence
• Minimum 1–2 years of medical-surgical or acute care experience
• Minimum 1 year of home health experience
• Orthopedic/post-joint replacement experience required
• Current BLS certification and phlebotomy competency required
• Proficiency with Epic charting system

Preferred Experience / Skills

• Experience with OASIS documentation and home health productivity standards
• Background in post-stroke, pneumonia, or complex post-surgical patient populations
• Strong customer service skills and ability to represent a professional standard of care in a home setting
